• ID proof (Prefer Aadhar Card)

• Doctor Note

For Emergency IPD admission under Chiranjeevi scheme –

All you need to do is bring your Chiranjeevi policy document with you for admission in Emergency ward. Rest of the documents can be submitted after the admission in between a time interval.
