Process of OPD Consultation / Investigation and IPD Admission in ECHS

For OPD Consultation / Investigation in ECHS –

• You have to visit the ECHS counter of Shivam Hospital for OPD consultation / investigation under ECHS scheme.

• You have to show your ECHS card, ECHS referral and ID proof (Prefer Aadhar card ) at the Shivam Hospital counter.

• You can get a consultation/investigation slip by showing your ECHS card at the Shivam Hospital counter.

For IPD Admission in ECHS –

If you have normal IPD admission then you have to visit the ECHS counter of Shivam Hospital and you will need to show your –

• ECHS Card

• ECHS Referral

• ID Proof (Prefer Aadhar Card)

• Doctor Note / Relevant Investigation Slip.

For Emergency IPD admission in ECHS –

All you need to do is bring your ECHS card and Aadhar card with you for admission in Emergency ward. Rest of the documents can be submitted after the admission in between a time interval.
